Insurance Coverage
I am an out of network provider for insurance and can provide you with a monthly statement of services, upon request.
It is your responsibility to submit this form to your insurance company for reimbursement. Please verify your coverage for therapy services before beginning psychotherapy if you plan on submitting to your insurance company.
Cancellation Policy
24-hour cancellation notice required.
I understand that sometimes schedule adjustments are necessary. Therefore, I respectfully request at least 24 hour notice for cancellations or rescheduling of appointments. Please understand that when you forget, cancel, or change your appointment without giving enough notice, I miss the opportunity to fill that appointment for another client/family/couple to receive a therapy session.
Any appointment missed, late cancelled, or changed without 24 hour notice will result in a charge equal to 100% of the reserved session amount.
